Types of Dermal Fillers

Dermal fillers can be grouped into two types:

Hyaluronic Acid Fillers

Contains hyaluronic acid (HA), a natural compound found in our skin. It is responsible for many functions in our body like hydration and lubrication of joints. It also aids tissue repair and stimulates the production of collagen.

Examples of HA fillers include Juvederm, Restylane, and Belotero.

Collagen-Stimulating Fillers

Contains safe, synthetically-produced compounds that when injected into the skin, volumises the area and stimulates the body’s fibroblasts to produce collagen for long-lasting effect.

Examples of collagen-stimulating fillers include Radiesse+, Sculptra, and Ellanse.

Firm or soft? What do I need?

There are many different filler brands available and each brand contains a line of different dermal fillers, which are differentiated based on the firmness and viscosity (thickness and stickiness) of the gel.

Whether you need a firmer or softer dermal filler will depend on the area that requires treatment, the condition of your skin, and the desired result. If your skin is looser, for instance, a softer filler (e.g. Juvederm Volbella, Belotero Soft, Restylane) may be used for a more natural look.

On the other hand, a dermal filler that is firmer and higher in viscosity (e.g. Juvederm Voluma, Belotero Intense, Radiesse+) provides a good underlying foundation to fill up facial volume loss, support sagging skin, and allows the doctor to sculpt or enhance specific facial structures like your cheekbones or nose.

Why is technique important?

The technique used to apply the dermal filler is more important than the type of dermal filler. Doctors are moving away from needles to using micro-crannula to apply fillers.

A micro-cannula is a blunt flexible wire that doctors insert and slide under the skin to apply the dermal filler. There are many advantages to using a micro-cannula:

It gives a natural and more even fill.

Bleeding and discomfort is minimised.

Lowers the chance of complications including bruising.

Safer with fewer possible side effects compared to needles.

One injection point compared to multiple injections.

Customised dermal filler treatment to suit the individual's needs.

Dermal Filler FAQs

Most facial fillers contain lidocaine, an anaesthetic, which will numb the treatment area to make the experience a comfortable one. The doctor will also use multiple treatment protocols like the use of the micro-cannula, numbing cream, and ice packs to make you comfortable.

It varies as different people have different volume requirements and expectations. Let your doctor advise you during consultation.
 

After cleansing and numbing your face, your doctor will insert the filler with a micro-cannula. Treatment takes about 30 mins to 1 hr depending on your treatment area.

 

Yes, dermal fillers can be combined with other treatments. Patients commonly combine dermal fillers with botulinum toxins to enhance their face rejuvenation or facial contouring results. Dermal fillers can also be paired with Ultherapy for more dramatic anti-ageing results. During your consultation, our doctors will create a customised treatment plan for you to check your suitability.

The longevity depends on the type of filler used and the treatment area. It can range from one year to two years, and maybe more for collagen-stimulating fillers.
 

No special care is required. However, if possible, avoid applying make-up over injection site for up to 4 hrs to prevent infection. Alcohol, Aspirin, and non-steroidal anti-inflammatory medications (i.e. painkillers) should also be avoided to reduce the chance of bruising. Light exercise is fine, but it is recommended that you avoid vigorous exercise and rubbing the treated areas for two days following treatment to prevent infection and to give time for the fillers to settle down so that it will not move out of place.

 
Occasionally, reactions can occur, but they are temporary and typically resolve within a week. This include bruising, redness, swelling, tenderness, and slight bleeding at injection sites. For the best recovery, follow your doctor’s instructions.

Dr. Mavis Pak’s journey into the realm of aesthetics was sparked by her early passion for art, nurtured during her GCE A Levels Art studies nearly two decades ago. This lifelong fascination in the Arts seamlessly integrated into her medical career, shaping her into the adept practitioner she is today. To achieve natural-looking results, Dr. Mavis Pak meticulously assesses each individual’s facial structure, prioritising the restoration of lost volume. Utilising bio-stimulating fillers, she harmonises facial features to create balance and symmetry. 

When it comes to facial sculpting with dermal fillers, she focuses on restoring facial volume first with the use of bio-stimulators. “My preference is to use bio-stimulating fillers, also known as collagen-stimulating dermal fillers as they deliver more natural looking results, and they help to restore volume loss which is my primary concern,” says Dr. Mavis Pak.

With a commitment to natural beauty, Dr. Mavis Pak specialises in facial sculpting with dermal fillers. Her preference for bio-stimulating fillers stems from their ability to deliver results that are not just noticeable but transformative, addressing volume loss. Central to her approach is the technique of filler layering, honed through years of experience and dedication to her craft.

Her pursuit of knowledge extends further with a Post Graduate Diploma in Clinical Dermatology with Distinction (UK) and a Post Graduate Diploma in Aesthetic Medicine (USA).

Dr. Terence Tan

Dr. Terence Tan is the founder of Halley Medical Aesthetics and Halley Body Slimming Clinic. He has a Bachelor of Medicine, Bachelor of Surgery, and Master of Medicine (Surgery) from the National University of Singapore (NUS). He also obtained his surgical qualifications with the Royal College of Surgeons of Edinburgh in 2003.

Under his leadership, both medical clinics received multiple awards including the “Golden Record Award” by Merz Aesthetics in 2016, 2018, 2019, 2020, 2021, 2023 for Ultherapy treatment and the “Merz Aesthetics Singapore Ultherapy Transducers Winner” award in 2022. In the area of anti-ageing and facial contouring with dermal fillers, Halley Medical Aesthetics won Allergan Aesthetics’ Facial Aesthetics Gold Award in 2022 and 2023 as well as Galderma Aesthetics’ Alliance Awards in 2022 and 2023. Both Allergan Aesthetics and Galderma Aesthetics are creators of multiple dermal filler brands.

Dr. Terence Tan is actively involved in research that will advance the care and treatment outcome of medical aesthetic treatments. He has a special interest in the use of cryolipolysis or freezing temperature to remove unwanted body fat for body contouring. His research has been presented in international meetings from Monaco and Spain to Japan, China, and the Middle East.

When it comes to dermal filler treatments, Dr. Terence Tan says that a lot of people are concerned about “overfilling” and their doctor’s technique on achieving natural-looking results. “Many patients want to look fresher and better without people asking them if they had done something to their face. But at the same time, patients also want the treatment to address their skin concerns,” says Dr. Terence Tan.
